All REGISTRATIONS must be completed by June 21st.  Late REGISTRATIONS will be accepted up through the start of Summer Music camp for an additional $15 fee.

  • Please complete ONE registration form per family member attending, however submit only one check.

  • Please note you will pay for the cost of the entire session, no matter how many days you attend

  • Once in attendance, there will be no refunds

  • You will be contacted at a later date regarding your ensemble placement

  • ALL CHECKS MUST BE MADE OUT to: WESTVIEW FOUNDATION

  • All REGISTRATIONS MUST be complete by June 21st

  • Mail the Medical Waiver by June 21st to the address listed below:    (or drop off at Mesa Verde Middle School NO LATER THAN JUNE 21)

For multiple family members, please complete separate registration forms but pay with one check
